Deep buttock pain that radiates down the leg is frequently diagnosed as sciatica or piriformis syndrome, but trigger points in the gluteal muscles, piriformis, and deep hip rotators are often the primary culprits and are entirely addressable without medical intervention.
The piriformis, gluteus medius, gluteus minimus, and tensor fasciae latae are the key trigger point sites for this pain pattern. Gluteus minimus trigger points in particular refer pain in a near-identical pattern to true sciatica, making misdiagnosis common.
Trigger point-driven leg pain typically has a more diffuse, aching character and does not follow a precise dermatomal distribution. It may shift with position and often responds immediately to direct pressure on the source muscle.
Begin with the piriformis in the midpoint of the buttock, then move to the gluteus medius along the outer hip crest. Apply sustained pressure to tender points for 60 to 90 seconds each. Many users see significant improvement within the first two to three sessions.
